Tirzepatide sales drive a Lilly guidance raise as Mounjaro grows 65 percent

Mounjaro and Zepbound brought in about 16.1 billion dollars in the third quarter, and Lilly lifted its 2026 revenue range to 91 to 92 billion dollars. Tirzepatide stays approved on the board, with lower prices and pill rivals as the open questions.

The readTirzepatide does not move on the board, but the quarter shows its commercial lead widening: volume grew fast enough to outrun lower prices in China and the US, and the label gained a cardiovascular claim in type 2 diabetes. The revenue raise is a company forecast, and the report adds no new clinical data or persistence figures.

Eli Lilly's third quarter report, published on 29 October 2026, shows tirzepatide sales still growing at a pace few medicines reach at this scale. Mounjaro and Zepbound together brought in about 16.1 billion dollars in the quarter, roughly two thirds of total revenue, and Lilly raised its full year revenue range to 91 to 92 billion dollars. On our board, tirzepatide stays at the approved stage. The move is commercial rather than clinical, but it sets the bar that rival incretins, amylin combinations and oral GLP 1 pills now have to clear.

Tirzepatide sales carry the quarter

Lilly reported total revenue of 24.4 billion dollars for the quarter, up 39 percent on the same period of 2025, according to the company's release. Mounjaro, the tirzepatide brand for type 2 diabetes, earned 10.7 billion dollars, a 65 percent rise in Mounjaro revenue. Zepbound, the obesity brand, earned 5.35 billion dollars, up 49 percent. A footnote in the release says the Zepbound name is used for obesity only in the United States, Canada and Japan, so the Mounjaro total likely includes some obesity use in other markets. The financial news site 24/7 Wall St. reported that worldwide tirzepatide volume rose 53 percent year over year, with international demand leading. Taken together, tirzepatide sales made up about 66 percent of everything Lilly sold in the quarter.

Volume and price moved in opposite directions. 24/7 Wall St. linked part of the international gain to the listing of Mounjaro on China's National Reimbursement Drug List, which widened access but lowered the price Lilly realizes. In the United States, CNBC reported that Lilly's revenue rose 34 percent to 15.1 billion dollars on a 41 percent rise in volume, driven mainly by the two tirzepatide brands. The gap between volume growth and revenue growth is the price effect at work, and Zepbound sales grew more slowly than Mounjaro in the quarter. The table below sets out the figures Lilly published for its incretin products, including Foundayo, the brand name for orforglipron, its once daily non peptide GLP 1 pill, which was not yet on sale a year earlier.

Lilly revenue guidance now starts at 91 billion dollars

Lilly raised its 2026 revenue guidance to a range of 91 to 92 billion dollars, from 85 to 87 billion dollars before this report. The company opened the year in February with a range of 80 to 83 billion dollars, so the floor has climbed by 11 billion dollars in about nine months. Chief executive David Ricks credited global consumer demand for Mounjaro and Zepbound, wider obesity coverage in the United States and a doubling of revenue in oncology, immunology and neuroscience, according to 24/7 Wall St. Call summaries describe the outlook as volume driven, with management expecting continued pressure on price. The new Lilly revenue guidance is a company forecast, and it depends on demand holding up through the fourth quarter.

The company opened the year in February with a range of 80 to 83 billion dollars, so the floor has climbed by 11 billion dollars in about nine months.

The profit figures need more care than the sales line. Non GAAP earnings came to 7.48 dollars per share, a figure that already absorbs 4.57 dollars per share of acquired research charges from business deals, including the AtaiBeckley purchase, according to CNBC and the earnings call. Reported earnings were 7.22 dollars per share. Revenue also gained from one time Jardiance sales milestones, and Lilly disclosed that none remain, 24/7 Wall St. reported. Coverage differed on the direction of the per share profit outlook: CNBC put the new range at 35 to 36 dollars against a prior 35.50 to 36.50 dollars, while 24/7 Wall St. described it as lifted. The revenue raise, by contrast, is large and not in dispute.

Onswik approval and a new cardiovascular label

The quarter also widened the tirzepatide label. On the earnings call, Ricks said the FDA approved tirzepatide to reduce cardiovascular risk in adults with type 2 diabetes. The call excerpts we reviewed did not restate the trial data behind that decision, and the updated prescribing information is the place to check its exact wording. Lilly also listed the Onswik approval: the FDA cleared insulin efsitora alfa, a once weekly basal insulin injection for adults with type 2 diabetes. That puts Lilly up against Novo Nordisk's weekly insulin icodec in the same weekly basal niche. The release reported no Onswik revenue line yet, so its uptake cannot be judged from this quarter's numbers.

The next tirzepatide based molecule is already on the board. EloraTZP pairs tirzepatide with eloralintide, Lilly's amylin analog. On the call, Lilly said the combination produced 23.3 percent weight loss at 48 weeks in Phase 2, which it described as 57 percent more than tirzepatide alone in people with obesity and type 2 diabetes. 24/7 Wall St. reported that EloraTZP Phase 3 trials begin in the fourth quarter of 2026. We covered the two AMPLIFY trials when they were registered with 3,115 planned participants over 100 weeks. Phase 2 also showed more people stopping for side effects than on tirzepatide alone, a point the larger trials will need to settle.

What the quarter does not settle

Revenue measures demand and access, not clinical benefit, and this report adds no new efficacy or safety data for tirzepatide. Lilly did not publish how many patients stay on treatment for a year, although persistence in obesity came up during analyst questions on the call. Price is the other open variable. The China listing and wider coverage in the United States bring in more patients at lower net prices, and the guidance assumes volume keeps outrunning those cuts. That assumption has held this year, but it is not guaranteed. The product tables sit in the quarterly results filing that Lilly lodged with the SEC on 29 October, which lists revenue by brand but not patient numbers or net price per patient.

The quarter also frames the race with pills. Foundayo, Lilly's own oral GLP 1 drug, brought in 184 million dollars, below the roughly 229 million dollars analysts expected, according to CNBC citing StreetAccount estimates. Novo Nordisk's oral semaglutide is its direct rival. Foundayo is a small molecule rather than a peptide, so on this site its uptake matters mainly for what it does to injectable tirzepatide sales over time. In Europe, the CHMP opinion on orforglipron put the pill on course for a licence, with a Commission decision as the next step. For now, the weekly peptide injection supplies the bulk of Lilly's growth by a wide margin.

Conclusion

Tirzepatide sales of about 16.1 billion dollars in one quarter keep the GIP and GLP 1 agonist as the reference point for the incretin field. The board stage does not change, but the signal is up: volume grew fast enough to absorb lower prices, the US label gained a cardiovascular claim in type 2 diabetes, and EloraTZP is moving into Phase 3. The open questions are price erosion, persistence and whether pills take share. Lilly's next quarterly report is the next check on all three.

Watch for next

The start of the EloraTZP Phase 3 AMPLIFY trials in the fourth quarter of 2026, early Onswik uptake, and whether Lilly's next quarterly report shows tirzepatide volume still outpacing price cuts as oral GLP 1 pills such as Foundayo build share.
