Orforglipron CHMP opinion puts the Foundayo pill on course for an EU licence

The EMA committee recommended Lilly's once daily non peptide GLP-1 tablet for weight management and type 2 diabetes. Weight fell about 5 to 11 percent at 72 weeks, and the Commission decision and national pricing come next.

The readA clear regulatory yes for a second oral GLP-1 in Europe, and the first that is not a peptide. The weight figures are modest next to injectables, so Foundayo will compete on convenience, supply and price, and national reimbursement will matter more than the licence itself.

The orforglipron CHMP opinion moves Eli Lilly's daily GLP-1 pill one step from the European market. The European Medicines Agency's human medicines committee recommended on 15 October 2026 that Foundayo be authorised for weight management and for type 2 diabetes in adults. The European Commission still has to issue the licence, and prices will then be set country by country.

Foundayo is a tablet taken once a day. In its public summary of the decision, the EMA describes two uses. For weight management, it covers adults with obesity, or adults with overweight plus at least one weight related health problem, alongside diet and physical activity. For type 2 diabetes, it covers adults whose blood sugar is not controlled well enough, either alone when metformin is unsuitable or together with other diabetes medicines. Treatment starts low and is raised step by step according to response and tolerance. The agency stresses one practical point: the tablet can be taken at any time of day, with or without food. That separates it from oral semaglutide, which has strict rules on fasting and water.

The reason is chemistry. The EMA describes orforglipron as a small, non peptide molecule that switches on the GLP-1 receptor, the same target that injected peptides such as semaglutide act on. Because it is not a peptide, it does not need the absorption enhancer and empty stomach routine that oral peptide tablets require. It also lowers blood sugar by helping the pancreas release insulin when glucose is high and by reducing glucagon. The opinion sits on our board as a move inside the approved stage, because Foundayo is already authorised in the United States. What changes is geography. Europe would become the next large market for a pill that Lilly can make without peptide synthesis.

The evidence the EMA weighed

For weight management, the committee relied on two studies with 4,740 adults. One enrolled people without type 2 diabetes but with another weight related condition, and the other enrolled people with the disease. After 72 weeks, weight fell by about 5 to 11 percent depending on the strength taken, against about 2 to 2.5 percent on placebo. For diabetes, four main studies with 3,765 adults measured the change in HbA1c, a marker of long term blood sugar. Foundayo cut HbA1c by about 1.2 to 1.9 percentage points depending on dose, compared with 0.4 to 0.8 on placebo and 0.8 on dapagliflozin. Oral semaglutide, used as an active comparator in one study, lowered it by 1.2 to 1.5 points.

After 72 weeks, weight fell by about 5 to 11 percent depending on the strength taken, against about 2 to 2.5 percent on placebo.

Those ranges deserve a careful reading. The EMA reports them across doses and populations, so the lowest figures likely come from people with diabetes, who usually lose less weight on any GLP-1 drug. The summary does not separate the two weight studies, and it gives no dropout rates. On blood sugar, the comparison with oral semaglutide points to similar or slightly larger reductions with Foundayo, but the EMA figures alone do not show a statistical win. We covered a Lilly funded analysis on that question in orforglipron versus oral semaglutide, where Novo had a model pointing the other way. The most common side effects were nausea, vomiting and diarrhoea, mostly during dose increases.

The European oral GLP 1 contest

Foundayo would not be first in Europe. Nordic Life Science notes that the recommendation follows EU approval of Novo Nordisk's Wegovy pill, the first oral GLP-1 for weight management there. The two tablets differ in design. Novo's product is a peptide that needs careful dosing conditions, while Lilly's is a small molecule that can be produced in chemical plants and taken more freely. The weight loss figures in the EMA summary are modest next to the best injectable results, so the contest is likely to turn on convenience, supply and price rather than on raw efficacy. For readers who want the full set of drugs in this class, the incretin molecules on our board include both pills and the main injectables.

The orforglipron CHMP opinion came at the same meeting as a recommended label change for Novo's weekly insulin and semaglutide combination, which we reported as the Kyinsu CHMP opinion. That single meeting shows how quickly the European pipeline for metabolic and weight drugs is filling up. For Lilly, the European file also tests whether regulators accept a single oral product for both obesity and diabetes, which the committee did. The decision does not, however, settle reimbursement anywhere in Europe. Many European health systems limit or refuse payment for weight loss drugs, and a positive opinion does nothing to change those national rules.

What happens next

The opinion now goes to the European Commission, which normally adopts a decision within about 67 days of a committee recommendation. That points to an EU licence before the end of the year if the usual timetable holds, although the Commission sets its own pace. After that, each member state decides on price and reimbursement, which in practice decides how many people can use the drug. Germany, France and other large markets assess value separately, and obesity coverage varies widely. Lilly has not, in the sources we checked, given a European launch date or a price for the tablet in any country.

Several data gaps also remain, and some will only close once the licence is formally issued. The EMA summary gives ranges, not dose by dose results, and it says nothing about cardiovascular outcomes, which matter for long term use in obesity. The full European public assessment report, published after the licence, will add detail on safety, discontinuations and the size of each study once the licence is granted. Until then, the orforglipron CHMP opinion is best read as a clear regulatory yes on efficacy and safety, with commercial access still undecided. The next checkpoint is the Commission decision, followed by the first national pricing outcomes.

Conclusion

The orforglipron CHMP opinion recommends Foundayo for weight management and type 2 diabetes in European adults, on the basis of six main studies. It confirms the pill's place in the approved stage and opens a second large market. Watch for the Commission decision, the public assessment report and national pricing.

Watch for next

The European Commission decision, normally about 67 days after the opinion, the European public assessment report, and the first national price and reimbursement decisions.
