Retatrutide kidney trial lowers albuminuria while measured GFR holds flat

In the phase 2b TRANSCEND-CKD study, measured filtration did not differ from placebo at 24 weeks, but albuminuria, glomerular pressure and kidney fat fell. Retatrutide stays in phase 3, and the earlier estimated GFR gains now look like a weight loss effect.

The readA neutral primary endpoint with favourable markers. Measured GFR did not differ from placebo at 24 weeks, which suggests the earlier rise in estimated GFR reflected weight loss rather than better filtration. Albuminuria, glomerular pressure and kidney fat fell, but in a short trial and, for albuminuria, a small subgroup.

Eli Lilly's triple agonist retatrutide now has its first dedicated kidney readout, and the result splits in two. In the retatrutide kidney trial, called TRANSCEND-CKD, measured filtration did not change against placebo at 24 weeks, while albuminuria, estimated pressure inside the glomerulus and fat around the kidney all fell. The phase 2b data were shown as a poster at ASN Kidney Week 2026 in Denver. The molecule stays in phase 3 on our board, and this readout opens a kidney question rather than closing one.

What TRANSCEND CKD tested and what it found

TRANSCEND-CKD enrolled adults with a body mass index of at least 27 and established chronic kidney disease, with or without type 2 diabetes. Filtration at entry had to sit in a moderate band, and baseline measured filtration averaged about 53 in both arms. People were randomised to weekly retatrutide or placebo, with the drug raised in steps over the first 16 weeks and then held at the top study level until week 24. The analysis counted 75 people on drug and 71 on placebo, which matches the 146 enrolled on the Lilly trial registry record. The primary endpoint was the change in measured GFR by iohexol clearance, a direct test that does not depend on blood creatinine or cystatin C and so is less exposed to the body changes that heavy weight loss brings.

On that primary endpoint the retatrutide kidney trial was neutral. Measured filtration slipped a little in both groups, and the gap between arms was 1.4 units, with a confidence interval wide enough to hold a real loss or a real gain, so the study cannot exclude a modest effect in either direction. Renal blood flow and filtration fraction did not move either. The secondary markers told a clearer story. Among the roughly 30 percent of people with raised albuminuria at the start, the urine albumin to creatinine ratio fell 53.7 percent on drug and 8.8 percent on placebo, according to the HCPLive report on the poster. Weight dropped by 14 kg on retatrutide with no change on placebo, and systolic blood pressure fell 13 points more than with placebo.

Why measured GFR matters more than an estimate here

The design answers a specific doubt. An earlier phase 2 analysis in people with obesity, published by the same lead investigator in Kidney International Reports in 2025, found that retatrutide lowered albuminuria and raised estimated GFR. The investigators noted that such estimates can be skewed when body composition shifts. Creatinine comes from muscle, and when people lose a large share of their weight the blood level can fall even if the kidneys filter no better. TRANSCEND-CKD put that worry to a direct test. Both the creatinine estimate and the cystatin C estimate rose significantly against placebo at week 24, yet measured GFR stayed level. In plain terms, the estimates moved and the kidneys did not, which is the exact gap the investigators set out to expose.

In plain terms, the estimates moved and the kidneys did not, which is the exact gap the investigators set out to expose.

That finding is not a failure for a drug that hopes to protect kidneys. Protection in chronic kidney disease usually shows up as a slower decline over years, and a 24 week study with 146 people was never built to show that. What the result does change is how readers should treat the earlier estimated gains: as a likely side effect of weight loss, not proof of better function. Kidney endpoints are new ground for the incretin column on our board. An earlier kidney move we logged went to a complement blocker for two rare kidney diseases, a very different drug class. Retatrutide would need outcome data, not markers, to earn a comparable kidney claim.

Albuminuria and pressure inside the kidney

Albuminuria is the clearest signal in the poster. In people who started with raised levels, the placebo adjusted fall in the urine albumin to creatinine ratio was 49.2 percent, and the split from placebo reached statistical significance by week 16. The confidence interval was wide, running from about a 71 percent fall to about a 12 percent fall, which reflects a small subgroup of roughly three in ten participants. Albuminuria is an accepted marker of kidney damage and a common target in kidney trials, but it remains a marker. Weight and systolic pressure also fell sharply in the same weeks, so this readout cannot yet say how much of the drop belongs to retatrutide itself and how much to those changes.

The pressure findings point the same way. An MRI based resistive index in the renal artery fell with drug against placebo, and estimated intraglomerular pressure fell by 2.8 mm Hg more than on placebo, while renal vascular resistance was unchanged. Fat around the kidney shrank by 13.4 points against placebo and fat in the renal sinus by 6.5 points. The investigators concluded that the falls in systolic pressure, resistive index, glomerular pressure and albuminuria suggest a profile that could support kidney protection over the long term. That is their reading, framed as a possibility. One safety note sits in the same data: pulse rose by 9 beats per minute on drug, a known signal for this class that larger trials will keep tracking.

Where the kidney data sit in the retatrutide programme

Retatrutide is a weekly injection that acts on GIP, GLP-1 and glucagon receptors, and its main programme is already in phase 3. HCPLive lists weight loss of up to 28.3 percent at 80 weeks in TRIUMPH-1 and A1C falls of up to 2.0 points in TRANSCEND-T2D-1. In July, Pharmacy Times reported that Lilly plans a biologics license application in the first quarter of 2027, citing a company release. The kidney poster does not change that timeline or the stage of the molecule. It sits beside the filing work as a mechanistic study, the kind that shapes how a later kidney outcome trial is read rather than one that supports a label on its own. A longer study, TRIUMPH-Outcomes, is listed as testing cardiovascular and kidney outcomes and is not yet reporting.

Our read on the retatrutide kidney trial is a flat signal with a useful clarification. The study met its aim of showing whether filtration truly changes, and the answer at 24 weeks was no, in either direction. The supportive markers were favourable, but they are markers in a short trial with a small subgroup. Readers comparing this with other incretin molecules should keep the two kinds of evidence apart: estimates and surrogates on one side, hard events on the other. Our note on how we weigh stage moves explains why a readout like this leaves the board unchanged. The next checkpoints are a full peer reviewed paper with the subgroup detail and the first kidney results from TRIUMPH-Outcomes.

Conclusion

The retatrutide kidney trial gives a precise answer to a narrow question. Measured filtration did not move over 24 weeks, which means earlier estimated gains were most likely an effect of weight loss. Albuminuria, glomerular pressure and kidney fat all fell, which keeps the kidney case open without proving it. Retatrutide stays in phase 3 with a filing planned for early 2027, and kidney claims will depend on outcome data that are still to come.
