Icotrokinra UK approval follows the US for the first oral IL 23 receptor pill

The MHRA authorised Icotyde on 23 October 2026 for moderate to severe plaque psoriasis from age 12, seven months after the FDA. The notice gives no efficacy numbers yet, and NHS access still depends on a NICE appraisal.

The readA clean second approval on terms close to the US label, which shows regulators outside the US accepting the same phase 3 package for an oral IL-23 receptor peptide. The commercial test in Britain is NICE, not the MHRA, and the notice holds back the efficacy and safety detail until the product text appears.

The icotrokinra UK approval landed on 23 October 2026. Britain's medicines regulator authorised Johnson and Johnson's once daily oral peptide, sold as Icotyde, for moderate to severe plaque psoriasis in adults and adolescents. The decision follows US approval of the first pill that blocks the IL-23 receptor by seven months.

What the MHRA cleared

The Medicines and Healthcare products Regulatory Agency granted the licence to Janssen-Cilag Limited, the Johnson and Johnson unit that holds British marketing rights. The label covers adults and adolescents aged 12 and older who weigh at least 40 kilograms and are candidates for systemic therapy. That last condition matters. It places the drug in the same treatment slot as injected biologics and older oral systemic agents, not as a first step after creams. In its announcement, the regulator said icotrokinra selectively binds to and blocks the receptor for interleukin 23, a signalling protein that drives the chronic skin inflammation seen in psoriasis. The tablet is taken once a day, and the full prescribing text is due on the MHRA products site within seven days.

The decision rests on four phase 3 trials with about 2,500 people with moderate to severe plaque psoriasis. That group included 72 adolescents aged 12 to under 18. According to the MHRA, all four studies showed efficacy against placebo at week 16 in the populations studied. The agency did not publish response rates in its notice. Julian Beach, the MHRA's executive director of operations, said the authorisation "provides another treatment option for patients living with moderate to severe plaque psoriasis." He added that the safety of the medicine "will continue to be closely monitored," and asked patients and clinicians to report suspected side effects through the Yellow Card scheme. No restrictions beyond the age and weight limits were stated.

How it compares with the US label

The US label came first. Johnson and Johnson announced FDA approval on 18 March 2026 for patients aged 12 and older who weigh at least 40 kilograms and are candidates for systemic therapy or phototherapy. The company listed the pivotal ICONIC studies by name, including ICONIC-LEAD and two ICONIC-ADVANCE trials. Those two trials compared icotrokinra with placebo and with deucravacitinib, an approved oral psoriasis drug. In head to head studies, the company reported that about 70 percent of patients reached clear or almost clear skin and 55 percent reached a 90 percent improvement in their psoriasis score at week 16.

The British wording is slightly narrower than the American one, because the MHRA notice mentions systemic therapy but not phototherapy.

The British wording is slightly narrower than the American one, because the MHRA notice mentions systemic therapy but not phototherapy. Whether that gap survives in the final summary of product characteristics is not yet clear. On our board, icotrokinra was last logged at the filed stage after the US filing for icotrokinra in 2025. The icotrokinra UK approval is the move that takes it to approved in our tracker, even though the US decision came earlier. The molecule was discovered jointly by Protagonist Therapeutics and Johnson and Johnson, which holds worldwide development rights under a licence agreement. Protagonist therefore has a direct financial interest in uptake.

Oral IL 23 receptor peptides draw rivals

The approval lands in the same week that a rival oral IL-23 receptor peptide changed hands. On 22 October, Gilead Sciences said it would pay 1.275 billion dollars upfront for HCXT-2001, a macrocyclic peptide from Shanghai based PepStar Therapeutics that is still in phase 1. Gilead framed it as a possible daily oral treatment for inflammatory bowel disease. That is a field where icotrokinra is also being tested, alongside psoriatic arthritis, ulcerative colitis and Crohn's disease, according to Johnson and Johnson. A second large company paying that much for an early molecule aimed at the same receptor suggests investors now treat oral peptides as a credible format for this target, not a curiosity.

For psoriasis patients in Britain, the practical question is access through the NHS. An MHRA licence is only the first gate. In England, the National Institute for Health and Care Excellence decides whether the drug offers value at its price before routine NHS use, and the sources we checked do not give a date for that appraisal. The sources we checked also give no British price. Until those steps are complete, prescribing is likely to be limited. Readers following other peptides in this class can see the full list of immune peptide molecules on our board, which includes complement blockers and receptor antagonists.

What remains uncertain

Several facts are still missing from the British decision. The MHRA notice gives no efficacy numbers, no safety table and no list of warnings, and it says the full product information will follow within a week. Without it, readers cannot see whether the British label carries the same cautions as the US one, or how the adolescent data are described. The adolescent evidence is also thin in absolute terms. Seventy two teenagers is a small group on which to base long term safety conclusions for a drug that may be taken for years. Post approval monitoring through the Yellow Card scheme will matter more than usual here.

The bigger strategic unknown is whether icotrokinra can move beyond the skin. Johnson and Johnson is running trials in psoriatic arthritis and in both forms of inflammatory bowel disease, and results there will decide whether this is a niche pill or a platform. A gut indication would also put it in direct competition with Gilead's newly acquired molecule if that one progresses. For now, the icotrokinra UK approval is a clean regulatory step with a clear label and no surprises. The next useful data point is the published summary of product characteristics, followed by a NICE timetable and any European decision.

Conclusion

The icotrokinra UK approval gives Johnson and Johnson a British licence for its oral IL-23 receptor peptide, on terms close to the US label. It moves the molecule to approved on our board. Watch for the product information text, NICE timing and the inflammatory bowel disease trials.
