Enlicitide CKD analysis shows LDL cuts hold when kidney function falls

A pooled look at the CORALreef Lipids and HeFH trials finds the oral PCSK9 inhibitor lowered LDL as much in people with reduced filtration as in everyone else. Severe kidney disease and dialysis remain blank spots.

The readThe drug worked at least as well when kidney function was reduced, which fits a label with no renal adjustment. The low eGFR group is small, and severe disease and dialysis were not reported separately. Useful detail for an approved drug, not a new claim of benefit.

A pooled look at two Merck lipid trials gives the first detailed enlicitide CKD figures. In people with an estimated glomerular filtration rate (eGFR) below 60, the oral PCSK9 inhibitor lowered LDL cholesterol by 65.9 percent more than placebo at week 24, according to an abstract presented on 22 October at ASN Kidney Week 2026 in Denver. That matches, and numerically tops, the result in people with better filtration. Enlicitide is already approved in the US as Lipfendra, so the molecule does not change stage; the analysis adds detail for a group that often misses lipid targets.

What the pooled CORALreef analysis measured

The analysis combined CORALreef Lipids and CORALreef HeFH, two phase 3 trials that compared a once daily enlicitide tablet with placebo. Together they supplied 3,212 participants. Investigators split them by baseline kidney function: 2,924 sat at an eGFR of 60 or more, while 288 sat below 60, the usual line for moderate chronic kidney disease. The main measure was the change in LDL cholesterol at weeks 24 and 52. Other measures covered non HDL cholesterol, apolipoprotein B and lipoprotein(a) at week 24, the share of people reaching LDL goals, and adverse events through one year. Christie Ballantyne of Baylor College of Medicine is first author of the abstract, labelled TH-OR037, and Pharmacy Times reported the details on 23 October.

The kidney question has a practical edge. People with reduced filtration carry high cardiovascular risk, yet many fall short of lipid goals, and most already take several heart and metabolic medicines. A pill that works the same way whatever the eGFR is simpler to use in that group than one that needs adjustments. That is the enlicitide CKD question the abstract set out to answer. Its limits are worth stating early. The low eGFR group is small, under a tenth of the pooled total, and the abstract gave no separate results for people with an eGFR below 30 or for people on dialysis. It also did not report adverse event rates by kidney group, only a summary statement that safety was comparable to placebo in both.

Enlicitide LDL lowering held across eGFR groups

In people with an eGFR below 60, placebo adjusted LDL cholesterol fell by 65.9 percent at week 24 and by 64.2 percent at week 52. In those with an eGFR of 60 or higher, the figures were 62.3 and 56.8 percent. The low filtration group therefore did at least as well, and numerically a little better at one year, although the reports do not say whether the difference between groups was formally tested. Investigators also reported significant falls in non HDL cholesterol, apolipoprotein B and lipoprotein(a) against placebo in both groups, each with a p value below 0.001. Goal attainment on enlicitide was close to identical across the kidney split, as the table below shows for both common LDL thresholds.

The low filtration group therefore did at least as well, and numerically a little better at one year, although the reports do not say whether the difference between groups was formally tested.

Ballantyne summed up the result in a comment to Pharmacy Times: "It has shown both efficacy and safety in patients with reduced eGFR." He also called enlicitide a new nonstatin oral option that has outperformed current nonstatin therapies and comes close to the injectable antibodies against PCSK9 in efficacy. Those are his views on comparisons across separate trials, not head to head results. For a pipeline tracker the key point is narrower. Enlicitide LDL lowering did not shrink when kidney function fell, which removes one reason a prescriber might hesitate in this group. It does not settle what happens in advanced disease, where the pooled trials did not report enough people to give an answer either way.

What the Lipfendra label says about the kidneys

The prescribing information, as summarised by Pharmacy Times, helps explain why the result is plausible. Enlicitide is minimally metabolised and, after intravenous dosing, leaves the body mainly through glomerular filtration, with about 73 percent of a dose found in urine. Despite that, the label projects steady state exposure only 5, 11 and 26 percent higher in mild, moderate and severe renal impairment. In end stage renal disease, single dose exposure was 17 percent higher when the drug was given before hemodialysis and 75 percent higher when it was given after. The label treats none of these rises as clinically meaningful and sets no renal dose adjustment, which fits the enlicitide CKD results from the pooled trials.

The label also covers interactions that matter in chronic kidney disease, where patients often take several cardiometabolic drugs at once. According to the label, enlicitide did not change exposure to lisinopril, digoxin, warfarin, atorvastatin, levothyroxine or oral semaglutide. The larger practical issue is food. The label asks for the tablet to be swallowed whole first thing in the morning, before eating, with only water, black coffee or plain tea, and then a wait of at least 30 minutes before anything else. Taken 30 minutes after a meal, steady state exposure fell by 48 percent. Ballantyne said adherence to the morning routine was good in the trials and that the drug's long half life softens the effect of an occasional missed or fed dose.

Where the oral PCSK9 inhibitor stands and what comes next

On 16 July 2026 the FDA cleared enlicitide, alongside diet and exercise, to lower LDL cholesterol in adults with hypercholesterolemia, including the heterozygous familial form. The decision rested on placebo adjusted week 24 reductions of 56 percent in CORALreef Lipids and 59 percent in CORALreef HeFH. Our earlier story on the first large phase 3 readout covers how the molecule reached that point. As a peptide based pill aimed at PCSK9, it belongs with the other molecules on our blood and lipids page. The kidney analysis changes no stage. It fills in how an approved drug behaves in one large and underserved subgroup of patients.

What enlicitide has not yet shown is that lowering LDL this way prevents heart attacks, strokes or deaths. That question belongs to CORALreef Outcomes, a phase 3 trial that, according to its ClinicalTrials.gov record, has an estimated enrolment of 14,550 and is active but no longer recruiting. Until it reads out, the drug is an LDL lowering therapy and nothing more, and the kidney subgroup should be read the same way. A real test in severe chronic kidney disease or on dialysis would need data the pooled lipid trials cannot supply. Readers can follow every molecule in this class, and where each one sits, on our pipeline board, which we update as each new stage move is confirmed.

Conclusion

The pooled data answer one narrow enlicitide CKD question well: LDL lowering held up, and looked numerically stronger, in people with an eGFR below 60, with safety described as comparable to placebo. They leave open the picture in severe disease and dialysis, and they say nothing yet about heart outcomes. The molecule stays approved for LDL lowering, and the next checkpoint is the CORALreef Outcomes readout. This story is research news, not medical advice.

Watch for next

CORALreef Outcomes, the phase 3 cardiovascular outcomes trial with an estimated 14,550 participants, which will show whether lower LDL on enlicitide reduces heart attacks, strokes and deaths.
